Oral HPV testing

I have been getting a few inquiries from clinicians about HPV testing on Oral samples. Specifically, if a patient has a lesion or spot on the tongue, can we do HPV testing? I have researched it and even though research shows a connection between oral cancer and high risk HPV, there is no information about testing. As far as I can tell, the only way to do a test is on tissue (a biopsy).

Does anyone have even the slightest bit of info on this? Thanks!

I am unsure at this point what the benefit would be. I am unaware of any large scale studies with respect to patient management and outcomes. Without a patient management protocol, I am unsure what the benefit of testing would be at this time. This area of interest perhaps has been mostly in men with anogenital infection. A very recent study using PCR, Oral Surg Oral Med Oral Pathol Oral Radiol Endod. 2009 Sep 16. (Epub ahead of print), showed the following:

RESULTS: Only 3 patients presented with a clinically detectable oral lesion. Among them, just 1 was HPV positive by the molecular assay. In all of the other patients (27 out of 30), oral clinical lesions were not detected and scrapings were all negative for HPV infection. CONCLUSION: The prevalence of concurrent oral and anogenital HPV infection was very low in this study (1 out of 30).
