Can you respond to the updated CLIA regs regarding cytotech workloads, and how liguid based vs. imaged paps are counted and what the maximum slides/day are?

Our lab currently conducts 5-year retrospective reviews not only on SILH+ paps, but also on any tissue with a diagnosis of SILH+. Is the retroreview necessary for the tissue specimens?

i HAVE A QUESTION ABOUT THE 12.5 RULE. HAVE YOU EVER KNOWN FOR LABORATORIES TO MONITOR THEIR TECHS HOUR BY HOUR? CAN THE LAW BE INTERPRETED TO MEAN THAT YOU SHOULD SPEND 5 MIN ON EACH SLIDE?

It is interesting how difficult it is to find documented evidence of CLIA's Cytology Quality Controls of "screening" more than 100 slides/8hrs/day and counting liquid-base sides as 1/2 a slide. I found this recently: goto www.cdc.gov/mmwr(external link) On the top/right of screen find SEARCH box and enter 00016177 then find "Regs for Implementing the Clinical Laboratory..." scroll down to "Cytology Quality Control" This is the first time I've seen reference to 1/2 slides for liquid-base gyn cytology and making up to 200slides/8hr/day. The date of this document is ambiguous (to me) and seems to be 8/5/98. I do not agree with this assessment of 200slides/8hr/day but there it is ...

Actually, upon further research, searches such as "Regulations for Implementing the ClIA '88: A Summary" on the www.cdc.gov/mmwr(external link) site (Morbidity & Mortality Weekly Report) brings up similar results of Gyn Cytology screening numbers/8hr/24hrs day. Another search "Regulatory Closure of Cervical Cytology Laboratories: Recommendations" shows the formula for determining per cent errors of Gyn Cytology Pap Smears. 5% being the minimal standard.
